Continuum of Care Program

We pride ourselves on the ability to follow our patients through our Continuum of Core program, ensuring that we coordinate the medical needs of our patients as they transition through various levels of Core.

  • Each of our residents will be followed by our COC Coordinator who will assist with coordinating their medical needs as they transition through the various levels of care.

  • Our providers are credentialed in the major hospitals in the Tampa Bay area. This provides options for each patients' hospital needs.

  • Our providers are located in many of the Skilled Nursing Facilities throughout the Hillsborough county area; conveniently located near our patient's homes or loved ones.

  • As an added value, our COC's monitor each resident's progress at every level of care and are available to provide updates to their loved ones and/or caretakers upon request.
